Open Enrollment Request for Application
Burke Center is the Department of State Health Services
designated mental health Authority established to plan,
coordinate, develop policy, develop and allocate
resources, supervise, and ensure the provision of
community based mental health and mental retardation
services for the residents of Angelina, Houston, Jasper,
Nacogdoches, Newton, Polk, Sabine, San Jacinto, San
Augustine, Shelby, Trinity and Tyler Counties, Texas.
Burke Center (“Local Authority”) is seeking proposals
for the provision of pharmacological management and
cognitive behavioral therapy (counseling) services for
identified individuals with mental illness who seek
services at the Local Authority. The services requested
shall be performed in Angelina, Houston, Jasper,
Nacogdoches, Polk, and San Augustine County at facility
location(s) of Applicant. The Network Agreement
(contract) shall be for an initial term of 1 year,
beginning February 1, 2010, with the option to renew
annually.
The open enrollment period shall close on August 15,
2009 at 5:00 PM (CST) or upon receipt of enough
applications to meet the service capacity described in
the RFA, whichever comes first.
